• Screening includes free faculty selected x-rays and limited screening exam
• Patients selected qualify for treatment only on days of licensing exam
• Please bring one form of ID
• Please bring current list of medications
• Must have teeth to qualify
• Non-English speaking patients require a translator
Must be 18 years old or older
CANNOT be allergic to latex
